For Second Coursers

  1. Submit original copies of the following
  2. Transcript of Records with remark “Copy for JPC”
  3. Accomplished JPC Admission form
  4. Accomplished JPC Registration form
  5. Student’s Statement of Responsibility
  6. 2×2 ID Picture with white background
  7. Submit photocopy of the following
  8. PSA Birth Certificate (photocopy)
  9. PSA Marriage Certificate (photocopy)

For Transferees

  1. Submit original copies of the following
  2. Transfer Credentials including
  3. Transcript of Records with remarks “Copy for JPC”; and
  4. Honorable Dismissal
  5. Accomplished JPC Admission form
  6. Accomplished JPC Registration form
  7. Student’s Statement of Responsibility
  8. 2×2 ID Picture with white background
  9. Submit photocopy of the following
  10. PSA Birth Certificate (photocopy)
  11. PSA Marriage Certificate (photocopy)

Enrollment Procedure

Step 1

(Course Assessment)

for NEW STUDENTS

1.1 Download and fill-out the Admission form and Registration Form

1.2 Submit the following for evaluation to jpc.certs@gmail.com
   a. Accomplished Admission and Registration Forms
   b. Copy of Transcript of Records for evaluation purposes

1.3 Wait within five (5) working days to receive your course mapping.

1.4 Fill-out the Academic Enrollment Form in accordance with your course mapping.

For OLD STUDENTS

1.1 Fill-out the Academic Enrollment Form in accordance with your course mapping.

Notes:
     a. in cases of Dropped and/ or Failing Rating in previous term, the same must be enrolled first. There may be a need to request an updated course mapping.
     b. Students with “INC” rating may enroll in the succeeding term while in the process of complying the requirements in the courses with “INC” rating.
     c. All financial obligations in previous term must be fully settled before enrolling in the succeeding term.

Step 2

(Payment)

2.1 Download, fill-out, and sign the Statement of Student Responsibility.

2.2 Pay your tuition and other fees thru the following modes:

2.3 Send a copy of your signed Statement of Student Responsibility and deposit or transaction slip with your full name, email address and mobile number indicated on it for confirmation to finance.jpc.certs@gmail.com

Step 3

(Access)

3.1 You will receive thru email your login credentials

3.2 Read the Orientation file.

3.3 Download the “CERTS Educ Online” app applicable to your device.

3.3 Enjoy learning!
