1.1 Fill-out the Enrollment Interest Form
1.2 Download and accomplish the Student Application Form.

PAYMENT
2.1 Pay your tuition and other fees thru the following modes:

Step 3
SUBMISSION
3.1 Submit the following to online.graduateschool@gmail.com
a. accomplished Student Application Form;
b. copy of deposit or transaction slip with your complete name, email address, and mobile number indicated on it; and
c. digital copy of other Admission Requirements as indicated in the student application form.
3.2 Notify us, through our facebook page CERTS EDUCATIONAL SERVICES, of your submission made.
3.3 Upon approval of your enrollment by UPHSD, we shall forward to you your official Registration form indicated therein your courses enrolled.
